Write to their GP: letter builder

The words are hard to find when you're worried. Fill in the blanks and this puts them into a clear letter you can print or copy.

A GP may be unable to discuss a patient with you without consent, but they can listen to information you provide. Where possible, tell the person that you are writing. A letter can still be useful when that is not possible.
